Background: Direct–to–consumer genetic testing (DTCGT) offers risk estimates for a variety of complex diseases and conditions, yet little is known about its impact on actual users, including their decisions about sharing the information gleaned from testing. Ethical considerations include the impact of unsolicited genetic information with variable validity and clinical utility on relatives, and the possible burden to the health care system if revealed to physicians. Aims: The qualitative study explored primary care patients’ views, attitudes, and decision making considerations regarding (...) DTCGT. This article focuses on the disclosure decisions participants made regarding participation, testing, and results of DTCGT, a topic which arose as a secondary aim of the study. Methods: Through four longitudinal interviews (pre–test, results, 3 and 12 months post–test) we examined twenty primary care patients’ decisions, expressed intentions, and actions regarding disclosure to immediate and extended family, friends and coworkers, and physicians about participation in and results of DTCGT. Individual interviews were analyzed using qualitative content analysis and a summative approach to describe the global themes. Results: Most participants disclosed to some immediate family; less than half disclosed to extended family; approximately half talked to friends. Most participants stated they would or might disclose to physicians about DTCGT and a few did. Conceptual themes that emerged from the data analysis include ambivalence about disclosure, consistency between intention and actual disclosure behavior and decisions, and conditional information sharing. Conclusions: Participants’ intentional and actual disclosure patterns offer insight into how they view DTCGT, weigh results, and the potential impact of DTCGT. (shrink)

My original course for Long Term Care was four weeks long after that I took an additional two months at a hospital to work on a Med/ Surg floor. So, I have a combined three months of schooling.Personally, I'd like to see certified nursing assistants (CNAs) given (...) more responsibilities. I'd like to see a six-month program to include CPR and capillary blood glucose (CBG) training as well as more in depth anatomy training. I think six months would keep some, who are just looking for money, out of the field and help keep those of us who really want a career in nursing in the field. It would be a better stepping stone for those who plan to go on to a LPN or RN program.I currently have CBG training but my employer will not let me use it. It's disappointing because it's a skill I have that I can't use and it could really help out the nurses. I think if courses were a bit longer, CNAs might be taken a bit more seriously by their employers and respected by their colleagues in the medical field.I was trained in CBG while working for the hospital and received a card like a CPR card to show I was trained. I live and work and was trained in Arkansas. I'm not allowed to do CBGs in the long-term care and rehab facility where I work now, though. However, unless the resident is diabetic we do have to clip and file nails. It took many years for nurses to gain respect and be allowed to do some things that now CNAs do. For example, for many years doctors didn't feel nurses were qualified to even take vital signs. I suppose CNAs will have the same struggle. The same could be said for LPN training, could it not? Or even an RN. I don't understand why there is a train of thought that "anyone" should be allowed to be a CNA and as quickly as possible. I can tell when I'm working alongside a Certified Nursing Assistant who is in it because they want to be versus someone who wants the paycheck (not sure why, though, because to be honest Arkansas doesn't pay CNAs all that great, though I suppose for some it might be better than McDonalds). I took a $5 an hour paycut go get back into nursing because it's my passion, and while we all need a paycheck, that should not be the only reason for doing this work."If extending the initial training time would keep many out of the field because they need a job with income ASAP, maybe employer facility-specific follow-up training would seem to be much more feasible for all. A CNA-in-training could be responsible for a lighter than 'normal' shift assignment while under the guidance of a qualified proctor CNA. It could improve efficiency and quality of care in the long run and probably improve retention rate of CNAs, for those administrators that are astute enough to consider the long-term effects of their policies." This might be good—the only problem is that most facilities aren't going to want to absorb any cost this might create by having additional staff for this purpose. Also, on the job training is not appreciated by employers as much as what might be considered actual "education" hours. Most CNAs want to do as much patient care as they can for the nurse—learning to assist with dressing changes, CBGs and other things that would be beneficial for both CNAs and the Nurse.Ensuring there is enough staff and that those staff members are there for the right reasons would do much more to improve the retention rate of NAs or CNAs. Slicno tome, mnogi Srbi, koji su bili najveca grupa u bivsoj Jugoslaviji, s nostalgijom su gledali na vreme nacionalnog ponosa i materijalnog komfora. Nasuprot tome, potcinjene etnicke zajednice u isto to vreme osecale su frustraciju u teznji za nacionalnim ponosom. Svaki politicki poredak ima jednu pripovest sacinjenu od odabranih i povezanih dogadjaja koji promovisu (...) nacionalne interese. Iako se zavrsila oruzana borba, u bivsoj Jugoslaviji ostala je podeljenost koja je perpetuirana konkurentskim narativima o tome sta se i zbog cega dogodilo. In the transcendental aesthetic of the Critique of Pure Reason, Kant claims that space and time are neither things in themselves nor properties of things in themselves but mere subjective forms of our sensible experience. Call this the Subjectivity Thesis. The striking conclusion follows an analysis of the representations of space and time. Kant argues that the two representations function as a priori conditions of experience, and are singular "intuitions" rather than general concepts. He also contends that the representations underwrite (...) some non-trivial a priori cognition of the objects of sensible experience. The Subjectivity Thesis is then presented as an immediate consequence: Space represents no property at all of any things in themselves nor any relation of them to each other. . . . For neither absolute nor relative determinations can be intuited prior to the existence of the things to which they pertain, thus [neither] can be intuited a priori. Time is not something which exists of itself, or which inheres in things as an objective determination. . . . [Were it] a determination or order inhering in things themselves, it could not precede the objects as their condition, and be known and intuited a priori by means of synthetic propositions. Kant's argument for the Subjectivity Thesis seems to have the following structure: * No absolute or relational features of things in themselves can be cognized a priori. (shrink)

A central doctrine of Kant's Critique of Pure Reason holds that the content of human experience is rooted in an affection of sensibility by unknowable things in themselves. This famous and puzzling affection doctrine raises two seemingly intractable old problems, which can be termed the Indispensability and the Consistency Problems. By what right does Kant present affection by supersensible entities as an indispensable requirement of experience? And how could any argument for such indispensability avoid violating the Critique's doctrine of noumenal (...) ignorance? This essay develops a new solution to both problems, setting out from the continuity between Kant's early and mature views on sensibility and mind-world relations. Kant's early writings subscribe to an interactionist cosmology opposed to both Leibniz's preestablished harmony and Malebranche's occasionalism. The modern debate on mind-world relations shaping Kant's early cosmology points us to a widely recognized motivation for interactionism, turning on a constraint on agency within certain noninteractionist cosmologies. In particular, Kant's early conversion to a libertarian theory of freedom, together with his rejection of occasionalism, provides the basis for a compelling argument for the indispensability of world-mind affection relations. Extended to the transcendental idealist framework, the same argument reveals noumenal affection as an indispensable presupposition of some knowledge claims consistently upheld by Kant. This leads in turn to a satisfying solution to the Consistency Problem, showing that the doctrine of noumenal affection is not merely consistent with, but is partly motivated by, Kant's commitment to noumenal ignorance. Kant’s Critique of Pure Reason (KrV) presents a priori knowledge of synthetic truths as posing a philosophical problem of great import whose only possible solution vindicates the system of transcendental idealism. The work does not accord any such significance to a priori knowledge of analytic truths. The intelligibility of the contrast rests on the well-foundedness of Kant’s analytic–synthetic distinction and on his claim to objectively or correctly classify key judgments with respect to it. Though the correctness of Kant’s classification is (...) vital to his philosophical project, it has been vigorously challenged since he introduced his famous distinction of judgments. The aim of this essay is to explore several metaphysical motives contributing to Kant’s objective classification claim that have been underemphasized and in part overlooked in the large literature on the topic. The paper examines Kant’s views on divine foreknowledge of contingent truths, in particular truths concerning free actions of creatures. It first considers the shape this traditional philosophical problem takes in the transcendental idealist context. It then situates Kant’s views relative to three competing theories of foreknowledge discussed by Leibniz. These are Molina’s theory of middle knowledge, the Thomist theory of foreknowledge through divine predeterminations, and Leibniz’s own ‘possible worlds’ theory. The paper concludes that no consistent theory of divine foreknowledge emerges (...) in Kant’s philosophy. His discussions alternate between two inadequate and incompatible models. One is a post-volitional model suggested by his conception of the intuitive intellect’s relation to creation. Extended to creaturely free action, it is incompatible with his commitment to the relative autonomy of free action. The other is a version of Leibniz’s possible worlds solution; it cannot underwrite certain foreknowledge of determinate outcomes in a libertarian setting. (shrink)

Education as a practice in its own right (or sui generis practice) invokes quite a different set of ethical considerations than does education understood as a subordinate activity ? i.e. prescribed and controlled in its essentials by the current powers-that-be in a society. But the idea of education as a vehicle for the ?values? of a particular group or party is so commonplace, from history's legacy as well as from ongoing waves of educational reforms, as to appear a quite natural (...) one. So much is this case that the idea of education as a sui generis practice may seem a bit eccentric at first sight. Some preliminary work is called for then to render intelligible the claim that education is indeed a practice in its own right, and to illustrate the original starting point this gives for an exploration of educational ethics. In undertaking this preliminary work, central themes from two major sources are explored and reviewed: Richard Peters? well-known study Ethics and education and MacIntyre's After virtue. The suggestive merits of both works for advancing a sui generis understanding of education and its conduct are identified. But crucial occlusions are also highlighted in the arguments of both authors, the recognition of which might have enabled their thinking on educational matters to venture onto a different plane. The kind of thinking that emerges from these investigations as most promising for educational ethics is seen to differ in its key features from what the various branches of academic philosophy have to offer by way of ethical theory. This article is the second of a two-part investigation, the first part of which was published in Ethics and Education, vol. 5, issue 2, 2010, under the title ?Preface to an ethics of education as a practice in its own right?. Although it builds on the arguments of that ?preface?, this second part of the investigation can be read as a stand-alone essay. It begins with a brief review of a new subordination of educational practice achieved by a neo-liberal tenor (...) in international educational reforms in recent decades in Western societies. The practical context for the essay however is that failure of many of these reforms, like the failure of neo-liberal dominance in socio-economic policy, has given rise to emergent opportunities where inspirations for educational debate and policy-making are concerned. Arguing for the uptake of such opportunity, the ethical tenor of education as a practice in its own right is explored under four headings: (1) review and clarification of the inherent purposes of education as a practice; (2) investigation of educationally productive pathways that are characteristic of education as a practice in its own right; (3) elucidation of a recognisable family of virtues that arise from that practice itself; (4) exploration of the kinds of relationships through which these virtues, and their educational fruits, are nourished. ABSTRACT The question concerning the connection of scientific inquiry to democratic praxis is central to both Antonio Gramsci and John Dewey. They share a common philosophical origin in Hegel and are essentially both in the tradition of Left Hegelian thought. Likewise, their respective analyses of the forces obstructing democratic emancipation were sharply focused on the distortions of social life caused by economic agents cooperating under hugely unequal power relations. As Gramsci wrote from his prison cell from 1929 to 1937 in (...) Italy, Dewey went through his most fruitful philosophical period in the United States, including his writings on politics and democracy. They both found targets of critique by diagnosing the pathologies of public life resulting from the power of private capital interests in collusion with a co-opted representative body, the authoritarian crimes and attendant culture of fascism, and the theoretical rigidity of the Soviet Marxists. In addition, true to their Hegelian roots, they marshaled their critiques of the abstractions of liberalism and its attendant moral and economic theory by insisting on the embedded, cultural, and historically deep contexts in which emancipatory practice need take place. Martin Heidegger is, perhaps, the most controversial philosopher of the twentieth-century. Little has been written on him or about his work and its significance for educational thought. This unique collection by a group of international scholars reexamines Heidegger's work and its legacy for educational thought.

Explanatory accounts of emotion require, among other things, theoretically tractable representations of emotional experience. Common methods for producing such representations have well-known drawbacks, such as observer interference or lack of ecological validity. Literature offers a valuable supplement. It provides detailed instructions for simulating emotions; when successful, it induces empathic emotions. It too involves distortions, through emotion-intensifying idealization and ideological biases. But these also relate to emotion study. There are three levels at which literature bears on emotion research: (1) the individual (...) work; (2) generic and related patterns; and (3) properties found widely across individual works and genres. Even at the third, most general level, literature suggests potentially important hypotheses about our pleasure in emotional simulation and our need to share emotional experiences. (shrink)

Recent decades have witnessed an explosion in neuroscientific and related research treating aesthetic response. This book integrates this research with insights from philosophical aesthetics to propose new answers to longstanding questions about beauty and sublimity. Hogan begins by distinguishing what we respond to as beautiful from what we count socially as beautiful. He goes on to examine the former in terms of information processing and emotional involvement. In the course of the book, Hogan examines such issues as how (...) universal principles of aesthetic response may be reconciled with individual idiosyncrasy, how it is possible to argue rationally over aesthetic response, and what role personal beauty and sublimity might play in the definition of art. This article rehearses the critical theory of Craig Calhoun’s book on nationalism and applies his threefold typology of ‘project, discourse, evaluation’ to the peculiar case of modern Philippine nationalism. The Republic of the Philippines is a marine archipelago of over 7100 islands and 85 million people of various ethnic, linguistic and cultural identities. Because of its history of colonizations (Spanish, American, Japanese), the predominance of Christianity, and the lack of a unified or prestigious pre-modern religious, political or economic order, the (...) Philippines is frequently positioned as ‘in but not of Asia’. Because Filipinos are made acutely aware of their peculiar trajectory as a modern nation-state and precarious position within the region, the dominant discourses of ‘the Philippines’ have been too anxious to construct a genealogy of the modern nation-state as telos and to assert some form of ‘Filipino’ identity and spirit. Now that the nationalist moment of contemporary nation-states has passed with the fading of the post-Second World War ‘developmentalist state’ project and its usurpation since the 1980s by ‘the globalization’ project, an opportunity is afforded for a changing of our critical lens from searches for revolutionary projects of founding myths and ends, of romantic nationalisms, or of nostalgic returns to pre-modern idylls (whether ethnic or colonial). Accordingly, this article has three parts. Part One provides a brief synopsis of Calhoun’s arguments about nationalism. Part Two applies these critical insights to the case of Philippine nationalism, especially as it is expressed in the historical and social sciences. Part Three then introduces alternative ways of thinking about the Philippines as sites of cultural hybridity and traffic. (shrink)

In modern pluralist Democracies, church influence struggles with pervasive influences from elsewhere for the hearts and minds of the public. Cities are sites and crucibles of creativity and destruction. How we order and imagine ourselves is revealed by the visible forms of our built environments. Cities are the ultimate material expression of human desire and design. They are also forces of energy and fields of tension that structure our everyday imaginings and activities. How we move, think, act, interact, create and maintain our lives is bounded by what cities provide us. How we make common-wealth and differentiate ourselves from others also (...) determines what cities are and might be for us. This issue explores the urbanism that is Manila in all its vital contradictions. A mega-urban region of over 20 million souls, it is both dynamic and discordant, creative and destructive. Like most global mega-cities, Manila is dangerous to its own citizens – polluted, dense, violent, and poor, with inadequate and badly maintained infrastructure and public utilities, but it is also a fecund incubator of cultural ideas, projects and networks. This issue explore both sides of this Janus-faced city – its urbanism and its visual cultures. Two common threads run through the articles in this issue: first is the dialectic of private and public, and second is the cultural traffic that informs place-making across time through the constant movement of peoples around the world-system and in the ever-pragmatic syncretic and local appropriations of cultural ideas, traditions and artefacts into new configurations, stories and views. As these contributions take issue with visual culture, we have given pride of place to photos, images and artworks themselves. For, as our colleague, Eduardo de la Fuente, has observed in this journal’s pages (2010), the sociology of art is more than a socio-aesthetics. It is also a chronicle of the experience, performance and movement of aesthetic objects lodged in and across institutions, traditions and collective consciousness. (shrink)

This essay seeks to show that teaching and learning are to be properly understood, not as an undertaking carried out on the will of a higher power or party, but as a way of life with an integrity of its own, arising from its own integral purposes. The essay thus seeks to provide an understanding of educational practice and of educational thought that contrasts in key respects with Alasdair MacIntyre's understanding, though also with a some notable parallels. A largely forgotten (...) ‘Socrates of Athens’ is identified as furnishing the original inspiration for the understanding of education explored in the essay. Some influential modern (and postmodern) negations of this understanding are then reviewed. Arising from its investigation of teaching and learning as a singular kind of relationship, the essay concludes with a brief sketch of some virtues that might constitute the way of life in question, in its more active and its more reflective moments. The practice of social theory is too often given to celebrity hunting, the polemical vulgarizing of one’s putative enemies, or the precocious production of totalizing and redemptive theories purporting to rescue social theory from its perennial crises of meaning, naming and explanation. The constructive task of social theory, however, can be both more modest and productive when attention is given to its substantive concern to provide codes, narratives and explanations of modernity, in all its pluralist and democratic dimensions. This is (...) in effect the self-description of Jeffrey Alexander’s own work. This paper provides an empathetic account of Alexander’s approach to the practice of social theory via a synopsis of his collected essays in Fin de Siècle Social Theory (1995). In particular, it claims that Alexander’s critique of the reductionist propensities of Pierre Bourdieu’s macro-sociological theory is exemplary in its constructive cast as a systematic analysis of the universalizing contents of the conceptual and methodological claims themselves. Westbrook provides an epistemological argument for democracy which features Cheryl Misak's version of "truth aptness" in moral and political discourse. Importantly, practices of citizenship are also pointed to in providing the habits necessary to engage in inquiry that democracy requires. However, while the regulative ideal of Misak's epistemology includes pragmatic reflection regarding multiple possible answers to moral questions and fallibilism with regard to these answers, it is still unclear what paying the compliment of truth to these beliefs accomplishes in terms (...) of practice, except to mark an exclusionary and potentially unpragmatic moment for democratic inquiry. We examine the problem of deducing the geodesic motion of test particles from Einstein's vacuum field equations and its extension to include gravitational radiation reaction. In the latter case we obtain an equation of motion for a particle which incorporates radiation reaction of the electrodynamical type, but due to shearing radiation, together with a mass-loss formula of the Bondi-Sachs type.

This reflection on the topic of emancipation stems from an ongoing project in tune with a wider development in pragmatic philosophy. Specifically, the project aims to piece together some of the consequences of pragmatism’s reconstruction of the tradition of philosophical inquiry, from the angle of human imagination. More recently this project has taken a different direction, in light of our critical situation under intensifying anti-democratic forces in the US, but also in many parliamentary democracies. Emancipation from forces that undermine democratic (...) transformation is arguably a goal that anyone gathering under the banner of pragmatism shares. The use of the pronoun ‘our’ in modifying ‘critical situation’ above is intended. It points to the scope of the problem. The problematic situation of ‘intensifying anti-democratic forces’ that sets the agenda for pragmatic inquiry is most aptly termed ‘neoliberal global hegemony’. Neoliberalism is a much-used technical term and its meaning is hotly contested. For the purposes of this paper, then, I would like to lift out several features common to almost all parties in the contest to provide a definition. This description will then be employed for the purposes of determining the character of the contemporary social context in which emancipatory practices take place. Second, by tying this description of the ‘background’ of our practices to the primacy of practical reason thesis, and specifically the role of imagination in practical reason, the pragmatic conception of agency comes into relief. A pragmatic conception of this social context of agency, the contemporary neoliberal imaginary, contributes to articulating prospects for emancipatory practice in a non-abstract sense. An example of experimentalist democratic practices of emancipation responding to crises generated by neoliberal practices is provided by recent efforts in worker co-operatives in Argentina. (shrink)
